Who is responsible

Medio Zorg B.V. (Chamber of Commerce 93290659) is responsible for the data you share through this website. Send privacy questions to info@mediozorg.nl.

What data

Through the contact form: your name, email address, and if you fill them in your phone number, organisation and a short note. Also what you are getting in touch about. Please do not share medical information through the form; you discuss that with Frank in the consultation.

Purpose and legal basis

Frank uses this data only to contact you about your request. The legal basis is that you ask for that contact, as a step towards a possible agreement (Article 6(1)(b) GDPR).

How long

If your request does not lead to an appointment, your data is deleted within twelve months. If you become a patient, Frank keeps a medical record. That falls under the WGBO and is in principle kept for twenty years.

Who sees the data

Only Frank. The website runs on Cloudflare. Messages from the form are sent with Resend (a US service; the email is sent from the EU) to Frank's mailbox. Medio Zorg signs a data processing agreement with these parties. If you choose WhatsApp, WhatsApp's terms also apply. Your data is never sold or used for advertising.

Cookies

This website sets no cookies. For visitor statistics Medio Zorg uses Cloudflare Web Analytics. It works without cookies and does not track you across other websites.

Your rights

You can ask to see, correct or delete your data, and object to its use. Send a message to info@mediozorg.nl. If you disagree with how Medio Zorg handles your data, you can file a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
